Abstract
Background: Penile fracture is an underreported penile trauma in which a tear occurres in the tunica albuginea because of forceful bending of the erected penis. Flaccidity of the penis occurs immediately after the fracture with resultant swelling and ecchymosis of the penis “eggplant” deformity. Surgical intervention to repair the tear is highly recommended instead of conservative management to prevent erectile dysfunction. Patients and methods: The files of 51 patients admitted between march, 1997 and April 2007 in the department of urology, 7th April teaching (Hawari)hospital, Benghazi , Libya were studied regarding the age, marital status, mode of trauma and the operation performed. Result: 85% of patients were under 40 years of age and 76% were single. The mode of trauma was forceful bending of the erected penis in about two thirds of patients while sexual intercourse was the cause in less than one third of the patients . Two cases had urethral injury and almost all other patients had only a unilateral tear in the corpora cavernosa. All patients underwent surgical operation to repair the tear successfully. All patients were followed up for 3 months, and there were no complications reported to any of the patients. Conclusion : Penile fracture occurred mostly to single patients due to forceful bending of erected penis in contrast to the literature from the western hemisphere were coitus is the most common cause. Surgical intervention is the only management recommended in all the literature to prevent complications of this embarrassing trauma.
